In accordance with Texas Senate Bill 12 (SB12), a parent or guardian must provide written permission before a student may participate in any club. This form is required for all student clubs in Junction ISD, including curricular, co-curricular, extra-curricular and competitive clubs. Please fill out the form for your childs participation.
