Requirements

Diphtheria, Tetanus, and Pertussis (DTaP/DTP/DT/Tdap/Td) |
Five Doses of a diphtheria-tetanus-purtusis containing vaccine, one of which must have been received on or after the fourth birthday; however 4 doses meet the requirement if the 4th dose was given after the 4th birthday. |

| Polio | Four doses of polio vaccine one of which must have been received on or after the fourth birthday; however, 3 doses meet the requirements if the 3rd dose was given on or after the 4th birthday. |
